Extra support to manage your health

Premera Personal Health Support clinicians offer personalized, one-on-one support for a range of health needs, after a hospital stay, for a chronic condition, for mental health, and more. This benefit is included with your health plan at no extra cost. Get help with:

  • Finding resources for your health and recovery
  • Coordinating care and services to help get the best treatment
  • Navigating the complex healthcare system

How it works

Who is eligible?

Members who are enrolled in a Premera plan, and their caregivers, spouses, or domestic partners are eligible for Personal Health Support. Your doctor may refer you to the program, or we may see from your claims that you could benefit from extra support. Either way, we’ll reach out to you directly, or you can request assistance by calling or emailing us at the contact information below.

Our personal health support program gives personalized, one-on-one support. Whether you have a complex medical condition, a behavioral health condition, a recent surgery, hospital admission, or nearly any health issue.

Registered nurses, registered dietitians, and licensed mental health professionals with expertise in chronic condition management.

Get help with illnesses such as asthma, coronary artery disease, heart failure, chronic obstructive pulmonary disease, diabetes, depression, and more. The program educates and encourages healthy lifestyle changes and self-management strategies for better health. It’s voluntary, and you can leave the program at any time.

Stay connected with your personal health support clinician and get personalized resources anytime, anywhere.

  • Have a question about your health? Use Wellframe to send a secure in-app message to a personal health support clinician.
  • When you sign up for the app, you will be automatically enrolled in the Welcome program, where you’ll have access to a physical activity tracker, nutrition guidance, and more!
  • You can also take a health assessment to evaluate your health status, lifestyle habits, and risk factors for disease.

  1. From your smartphone or mobile device, tap the QR code. If you’re on a laptop, scan the QR code from your mobile device. Download the app.
  2. Open the app. Your access code should autofill. Here it is in case you need it: bluewellpbc (for iOS or Apple device, tap “Allow Paste”)
  3. Complete the sign-up process. You’ll need your Premera member ID (found on your ID card).
  4. Need help? Call Premera Personal Health Support at 888-742-1479, Monday through Friday, 8 a.m. to 7 p.m., and Saturday, 9 a.m. to 1 p.m.

  • The health assessment only takes 15 minutes to complete and is 100% confidential—your responses aren’t shared with your doctor or employer.
  • Upon completion, your results will be sent to a Premera Personal Health Support clinician who will review your answers and reach out to you if you could benefit from additional assistance to reach your health goal. Or you can request outreach in the Wellframe app or by contacting us at the information below.
  • When you sign up for the app, you will be automatically enrolled in the Welcome program to connect you with a Personal Health Support clinician and get you started on your journey for better health.
  • No extra cost. All included in your health plan.
