Premera Personal Health Support clinicians offer personalized, one-on-one support for a range of health needs, after a hospital stay, for a chronic condition, for mental health, and more. This benefit is included with your health plan at no extra cost. Get help with:

Members who are enrolled in a Premera plan, and their caregivers, spouses, or domestic partners are eligible for Personal Health Support. Your doctor may refer you to the program, or we may see from your claims that you could benefit from extra support. Either way, we’ll reach out to you directly, or you can request assistance by calling or emailing us at the contact information below.

Our personal health support program gives personalized, one-on-one support. Whether you have a complex medical condition, a behavioral health condition, a recent surgery, hospital admission, or nearly any health issue.

Registered nurses, registered dietitians, and licensed mental health professionals with expertise in chronic condition management.

Get help with illnesses such as asthma, coronary artery disease, heart failure, chronic obstructive pulmonary disease, diabetes, depression, and more. The program educates and encourages healthy lifestyle changes and self-management strategies for better health. It’s voluntary, and you can leave the program at any time.
Call 888-742-1479, Monday through Friday, 8 a.m. to 7 p.m., and Saturday, 9 a.m. to 1 p.m. Pacific Time, except holidays.
